Havering Health in partnership with our member practices provided clinical expertise support to seventeen nursing homes, which represents around approximately 1024 beds. The service is commission by Havering Clinical Commissioning Group.
In addition to the routine weekly rounds and support by our member practices to the care homes, we deliver in -house dementia training, have provided death verification training and accredited to staff, dementia virtual tour and training , developed the management of Infections guidelines for Primary Care covering the following:

Upper respiratory tract infection
Lower respiratory tract infection
Gastrointestinal tract infection
Urinary tract infection
Genital tract infections
Meningitis
Skin Infections
We are committed to continuing to support our care homes and practices, and as part of education and training, we facilitated Information sharing in partnership with local community provider (NELFT) about the Community Treatment Team covering the following key areas:
Post-fall injury assessment and management
Blood Test request form
Combined Pathology GP Request Form
Nurse influenza vaccination consent form
Foot examination and risk classification
Urine sample information
Hydration Guide
First Line ONS and starter packs information
The education and training programme were well received by care home staff and has not only enhanced our relationships with care homes but support better patient care.
Havering Federation successfully ran the Care home contract ensuring that all reporting requirements and KPIs were met. A key part of the service delivery was establishing and holding regular MDT meetings with attendees from our health partners (acute and community) and social care. Havering’s success with regards to the MDTs resulted in our MDT Model being used as a case study for BHR and the wider NEL federations. Care Plans [Co-ordinate My Care (CMC)] is another area which saw significant improvement. Havering was consistently in the top 5 boroughs in London in producing Care plans for the residents of Care Homes. The contract with Havering Health expired on 31st March 2021. The new contract is commissioned via PCNs and all our PCNs have chosen to sub-contract with Havering Health. This is a reflection of our positive working relationships with PCNs and member practices. Access to shared care record is currently not in place however we are working with the CCG to improve the digital access of records by al care providers.
