Spirometry is one of the essential lung function investigations in the diagnosis, assessment of severity and monitoring of respiratory conditions such as COPD and Asthma. It is to be used in conjunction with history taking (symptoms/smoking/ lifestyle) and other physical assessment to confirm lung diseases. To have valid diagnosis, diagnostic spirometry must be quality assured and only be performed by people who have achieved the ARTP (Association for Respiratory Technology & Physiology) qualification.
Havering Health manages the delivery of this service with specialised nurses who have undertaken the ARTP training. The nurses are supported by a clinical lead. The Nurses carry out a number of tests as part of their consultation, and these results are sent back to the GP via EMIS who completes the diagnosis process.
This service was established in August 2021 and we operate from 3 hub sites, Rosewood Medical Centre, Petersfield Surgery and Raphael’s House.
The FeNo (Fractional Exhaled Nitric Oxide) test helps to diagnose asthma. As the patient breathes out it shows any inflammation in airways. This diagnostic has been running successfully alongside spirometry since May 2022.
Through the extension of clinics, recruitment of nurses and utilisation of digital solutions we have managed to increase activity and reduce wait times from 2 months to less than 1 month.
Patient Feedback on the service:
“You cannot improve perfection”
“The nurse was brilliant, called me straight in. Went through everything brilliantly. Showed me and talked through everything. She was fantastic, friendly. If I could give her 10 stars I would. Thanks for a brilliant service.”
“Could not have been bettered.”
GP Access
Havering Health in partnership with our member practices provided clinical expertise support to seventeen nursing homes, which represents around approximately 1024 beds. The service is commission by North East London Integrated Care System.
We are committed to continuing to support our care homes and practices, and as part of education and training, we facilitated Information sharing in partnership with local community provider (NELFT) about the Community Treatment Team covering the following key areas:
- Post-fall injury assessment and management
- Blood Test request form
- Combined Pathology GP Request Form
- Nurse influenza vaccination consent form
- Foot examination and risk classification
- Urine sample information
- Hydration Guide
- First Line ONS and starter packs information
The education and training programme are well received by care home staff and this has enhanced our relationships with the care homes and supports better patient care.
Havering Health Federation successfully runs the Care home contract ensuring that all reporting requirements and KPIs are met. A key part of the service delivery is establishing and holding regular MDT meetings with attendees from our health partners (acute and community) and social care. Havering’s success with regards to the MDTs resulted in our MDT Model being used as a case study for BHR and the wider NEL federations. The completion of Care Plans [Urgent Care Plans] is another area which has seen significant improvement.
